Care as unique as you at Korumburra Medical Centre

A HEALTHCARE professional who has moved to Korumburra to take up a new role to deliver a unique care service to older patients said the relocation to a rural clinic has given her a fresh perspective on nursing. 

Melanie moved from the Mornington Peninsula after accepting the role as the Practice Nurse and Care Manager for Mark & Sylvie’s Home Care at Atticus Health’s Korumburra Medical Centre. 

Mark & Sylvie’s offers an integrated model of care, with a team of GPs, nurses, Allied Health staff and support workers delivering holistic care to patients, allowing them to avoid hospitalisation and stay healthy at home. 

“It could be thought of as a one-stop shop for care. The benefit of this model of care is the support you receive which helps maintain your health and your independence,” said Melanie. 

As Practice Nurse and Care Manager, Melanie and her team take a proactive approach to chronic disease management and care planning, meaning that a client’s health is cared for, while their care needs in the future are also anticipated. 

“I work with each client and their family to develop a home support plan that suits their individual needs from arranging cleaners and gardeners to someone to help them get to medical appointments, personal care or even just some shopping and company.”

She urges anyone concerned about their health as they age to start planning now: “Don’t put off asking for help. The process of obtaining a package through My Aged Care takes time. Plan while you are still feeling well so that support is in place for when you don’t.”

Melanie and her partner grew up on the Peninsula but decided on a lifestyle change when it was time to buy a home.

“We wanted to find somewhere with a welcoming community vibe. After lots of exploring around the Gippsland region, we settled on Korumburra.”

She said the community has been welcoming and friendly, and the views are breathtaking.

“We’ve loved attending the local show and the garlic festival. We’re in the process of trying out all the local restaurants and take aways and its always a bonus that there is parking!”

Melanie has this advice for other health professionals considering a move to Korumburra or other regional areas: “Do it! There is so much to learn, a lot of variety, and a real sense of helping people rather than just ticking boxes in a system.”
